- Mandatory Details: Requires a 21-day travel history, exposure history, and any related symptoms before receiving immigration clearance.
- Real-time Sharing: The system immediately shares data with the Airport Health Officer, Bureau of Immigration, and State Surveillance Officers to quickly identify and refer at-risk travelers.
- Seamless Experience: Eliminates the need to fill out physical forms after landing.
How to Submit the Form
- Timeline: Passengers must complete the Self-Declaration Form (SDF) up to 24 hours before their arrival.
- Submission Window: Travelers are encouraged to fill it out during web check-in or before boarding their flight.
- Verification: Arriving passengers simply present the downloaded SDF at the International Travel Health Desk or Immigration counter.
